Submission Requirements


For new Submissions, the following information will be needed:

  • Completed and signed Acord Application
  • Completed Supplemental application, when applicable
  • Completed SOV spreadsheet
  • Photographs or web address that shows photographs of structures
  • Loss runs for the last 3 years
  • Copies of declarations pages from current program
